Dear Governor Hull:
This request for public records is made by the Arizona Chapter of the Association of American Physicians and Surgeons, a non-profit organization, pursuant to A.R.S. §39. This request is made for "non- commercial" purposes. As mandated by A.R.S. §39-121.01.8.1, we are making this request to the head of the office. The statute requires you to send this request immediately to the officer who maintains custody of your public records and to provide immediate access to the pertinent records, pursuant to ARS §39-121 et seq. Please let us know whether copies of documents will be sent by mail, and if not, the names and telephone numbers of individuals we need to contact or other procedures to be followed in gaining on-site access to review documents.
Please provide records relating to private or public grants received or provided by your office related to public health, health insurance, the delivery of health care, or the planning or oversight of health services or resources. These records include, but are not limited to:
(a) grant applications
(b) site visits for evaluating the application
(c) correspondence, meeting schedules and minutes, conferences, e-mail, facsimile communications, memoranda, telephone logs, or other documents pertaining to the conditions for receiving or providing such grants, including, but not limited to, the qualifications of contractors or the specifications for any contracts related to the implementation of the grant
(d) grant documents
(e) reports sent to or received from the granting agency concerning the planning, conduct, or evaluation of grant- related activities
(f) contracts or memoranda of understanding related to the current grant or to future funding of related projects
(g) payroll records for any employees or contractors of your office paid in whole or in part by grant funding
(h) notifications of and minutes from, memoranda concerning, or other documents pertaining to meetings of state administrators, state employees or contractors, legislators, legislative staff or committees, and personnel of the entity making or receiving a grant
(i) the mission statements of grantors and grantees
(j) contracts or memoranda of understanding related to the use of any state facilities or staff for the administration or implementation of grants
(k) contracts, memoranda of understanding, correspondence, or other documents pertaining to the use of employees, contractors, or facilities of the granting agency to carry out state functions pertaining to the grant
(l) correspondence, memoranda, contracts, minutes of meetings, telephone logs, or other documents pertaining to grants that may help to fund the State Children's Health Insurance Program (S-CHIP) program now being considered by the Legislature and the Turning Point Project
(m) Any correspondence, memoranda, telephone logs, publications, or other documents pertaining to any relationship between your office or its employees or contractorsÄand the office, employees, or contractors of other agencies that are under your controlÄwith the Robert Wood Johnson Foundation, the PEW Charitable Trust, the W.K. Kellogg Foundation, the Florida Healthy Kids Corporation, the Healthy Kids Replication Program, the Arizona Chapter of the American Academy of Pediatrics, Reach Out: Physicians' Initiative to Expand Care to Underserved Americans, the Alpha Center, Mathematica Policy Research, Inc., the Children's Action Alliance, Children Now, the AZHHA Education Foundation of Tempe, or other private foundations or organizations that provide or receive grant funding to or from your office, or contract with you to provide grant-related services
If any documents pertaining to our request are withheld, please provide the statutory basis for withholding them.
We agree to reimburse reasonable copying charges, up to a maximum of $500, if required by your office.
